By submitting this form and registering, I give the student camper permission to participate in the Atlanta Violins Upbeat! String Camp. The Undersigned acknowledges that the Atlanta Violins Upbeat! String Camp and its employees do not provide any registrant medical or hospitalization insurance whatsoever and hereby waives any and all claims against Atlanta Violins, Upbeat! String Camp, its employees and any other person affiliated with Atlanta Violins or the Upbeat! String Camp Program for injuries sustained while watching or playing in the camp or traveling to and from the camp, or participating in any leisure time activity. The undersigned also gives permission for the camper registered herein to use the internet for camp activities. The undersigned also gives permission for Atlanta Violins and the Upbeat! String Camp to use pictures of the camper registered herein for promotional purposes. Each session subject to cancellation if minimum enrollment of 40 is not achieved.
